Learn more about what medical records you need to send to us and bring with you. Proper medical records are critical in making your dialysis treatment aboard successful. Here you'll find an itemized list of essential medical record you'll need to prepare and bring with you on vacation.

Medical records you need to send to us prior to your cruise.

The following Dialysis® at Sea medical forms and medical information are required from all dialysis patients cruising with us. This information is due in our office by the due date specified in your Dialysis at Sea medical packet:

  • Completed Dialysis at Sea medical forms
  • 12 lead EKG tracing within one year of cruise date.
  • History and physical within one year of cruise date.
  • Current labs.
  • Hepatitis b antigen within 6 months of cruise date.
  • 3 current individual dialysis treatment sheets.

Everything you need to bring with you on the ship.

Here you'll find an itemized list of essential medical record you'll need to bring with you on your cruise:

  • Current monthly labs
  • Your last three dialysis treatment sheets prior to your cruise
  • Medications used while on dialysis (ex. Epogen, Zemplar, etc., including oxygen) because we do not provide them but will administer them. We do provide heparin and normal saline.
  • Personal medications
