Medical Records

The following medical information is required from all dialysis patients cruising with us:

  • 12 lead ekg tracing within one year of cruise date
  • History and physical within one year of cruise date
  • Current labs
  • Hbsag within 6 months of cruise date
  • Current Medication Sheet
  • 3 current flow sheets

ALL PATIENTS MUST ALSO HAND CARRY THEIR LAST THREE DIALYSIS TREATMENT SHEETS, LATEST LABS, ALL PERSONAL HOME MEDICATIONS WITH THEM TO THE SHIP.
